Shing Wai Chan, in "Troubleshooting Deployment in Glassfish," has documented a few common deployment problems in Glassfish, how to determine if you're suffering from them, and how to fix them.
It's an interesting idea: what other such problems and solutions are available for specific application servers? Perhaps someone should work on a categorical reference. :)
